If you are a new patient of our practice, we encourage you to download and print out the new Patient Information, Medical History Information, Medications and Refill Policy, and Medication Information Release Form. Please fill these out and bring them with you prior to your first visit. This will ensure your appointment with us is timely and efficient. ​​​​
